    • Capacity to work independently under supervision
    • Clinical supervision and mentorship with a personal development plan
    • Comprehensive assessment, planning, implementation, and evaluation of individual care needs

    About the Role
    The Support Worker will join the Older Peoples Service Community Mental Health Team in the North of the city. They will work with Service Users with severe, enduring, and complex mental health needs, including mood disorders, psychosis, and dementia. The role requires working independently but under the supervision of senior team members. The successful candidate will provide care co-ordination, incorporating robust care plans and risk assessments. The role also involves working as part of a Multi-Disciplinary team to provide specialist mental health interventions.

    Key Responsibilities

    • Provide professional input into the Multi-Disciplinary team
    • Offer care co-ordination with comprehensive care plans and risk assessments
    • Demonstrate a range of therapeutic interventions
    • Implement relevant forms of care without direct supervision
    • Supervise procedures and act as a mentor to students and newly qualified staff
    • Regularly act as the duty person to triage referrals to the team
    • Maintain up-to-date records using electronic care systems
