    Requirements

    • Registered Mental Health Nurse qualification
    • Full UK driving license
    • Experience with dementia and complex mental health needs
    • Effective communication skills (both written and verbal)
    • IT literacy and experience with electronic care records
    • Knowledge of MHA, MCA, and DOLS

    About the Role

    The Mental Health Practitioner will join the Intensive Care Homes Treatment Team, working to support service users in care homes. This role focuses on preventing hospital admissions by managing various mental health conditions, including dementia, depression, anxiety, and psychosis. Responsibilities include assessing mental health presentations, providing education and support to care staff, and offering guidance on non-pharmacological and person-centred approaches. The practitioner will work closely with a multidisciplinary team, including occupational therapists, nurses, and Consultant Psychiatrists, to deliver high-quality care and support.

    Key Responsibilities

    • Conduct timely assessments of mental health and risk
    • Role model appropriate behaviors and techniques
    • Educate care staff and provide tailored support
    • Collaborate with other professionals and families
    • Develop and implement risk management plans
    • Act as a shift coordinator and triage referrals
    • Support service users’ transition from hospital to care home
