    Experience in care plans and mental health assessments. Knowledge of the criminal justice system and its impact on health. Strong communication skills and the ability to work collaboratively with multidisciplinary teams.

    About the Role

    The Senior Mental Health Practitioner will work within the RECONNECT services, a care after custody initiative focused on improving the continuity of care for individuals leaving prison or an immigration removal centre (IRC). The role involves identifying health vulnerabilities, developing care plans, and coordinating support services to ensure a safe and supportive transition into the community. This position is crucial in reducing re-offending by addressing the health needs of individuals transitioning back to society.

    Key Responsibilities

    Provide mental health and social care assessments for individuals released from prison. Collaborate with senior partners across prisons, local authorities, and healthcare services. Develop comprehensive care plans to address health vulnerabilities. Support individuals’ transition from prison to community-based services. Maintain regular face-to-face contact with individuals before and after their release. Work closely with prison-based healthcare teams to ensure holistic care.
