    About the Role

    As an STR Worker in the Sheffield Health and Social Care NHS Foundation Trust, the successful candidate will play a critical role in providing consistent and compassionate mental health support. They will act as a named worker for a designated caseload, building strong working relationships with service users to promote their independence and manage their care plans effectively.

    This role involves collaborating with a multidisciplinary team to support care planning, engaging in regular supervision, and participating in ongoing professional development. The STR Worker will also be responsible for maintaining up-to-date records, contributing to risk assessments, and supporting the development of others within the team. The ability to work flexibly and respond to the needs of service users in times of distress is essential.

    Key Responsibilities:

    • Serve as a named worker for a designated caseload, providing consistent support.
    • Collaborate with the Multidisciplinary Team (MDT) to implement and update care plans.
    • Participate in the development and review of Recovery Goals with service users.
    • Contribute to unplanned activities through same-day appointment slots.
    • Support the completion of Patient Reported Outcome Measures (PROMs).
    • Identify and escalate risk issues appropriately in times of crisis.
    • Assist in health promotion, including addressing physical health needs.
